HOW TO APPLY FOR IGNITION INTERLOCK:
Participants who have been revoked must complete the following steps:
- Pass the DWI knowledge test- Go to Lakeview Mall Tues and Weds 830 to 430 (Buffalo, MN) or your local testing center. Get Knowledge Test receipt; take with you.
- Apply for a new Class D driver’s license JUST GO TO DMV and:
- Submit receipt from Knowledge Test;
- Pay the $680 reinstatement fee;
Commercial Driving License (CDL) privileges are not allowed while participating in the Ignition Interlock Device Program. Appropriate testing and application for CDL privileges can only be renewed upon completion of the program/revocation period.
Sign and date the Ignition Interlock Participation Agreement
Get a Certificate of Insurance for the vehicle(s) to be equipped with the device. The VIN (Vehicle Identification Number) of the vehicle with the device must match the VIN on the Certificate of Insurance.
If required, sign and notarize the Special Review Awareness form. This is necessary if your driver’s license has been cancelled IPS (as opposed to revoked).
Select an ignition interlock provider RADIOACTIVE IN BUFFALO or (view a list of State of Minnesota Certified Vendors) and have the device installed. Participants may install the device at any time but must wait until they receive notification from Driver and Vehicle Services (DVS) before driving the vehicle.
Submit application paperwork (steps 3-5) to DVS by uploading the documents, mail, fax, email or in-person (drop-off). Uploading the documents is the fastest. An interview with a driver evaluator is not required.

Participants will receive their restricted license in the mail once DVS approves the application. BE SURE YOUR MAILING ADDRESS MATCHES YOUR DL ADDRESS, IT WILL BE SENT TO YOUR DL ADDRESS.

- Submit receipt from Knowledge Test;

- Apply for Work Permit.
Participants will receive their limited license in the mail once DVS approves the application. BE SURE YOUR MAILING ADDRESS MATCHES YOUR DL ADDRESS, IT WILL BE SENT TO YOUR DL ADDRESS.
